Each ACLS Megacode Scenario within the simulator will take you through situations that you will experience when you perform your ACLS Megacode Skills Station.
After you have completed the scenario, the ACLS simulator will grade your test, and you will receive instant feedback. This will help reinforce the ACLS Algorithm and improve your ability to respond in emergency situations. Go through each megacode scenario as many times as you need to achieve mastery of the ACLS Algorithms.
After you have completed these multiple-choice megacode scenarios, move on to the megacode series videos which will take you one step further in your mastery of the ACLS algorithms.
The ACLS megacode simulator is compliant with the 2020-2025 AHA ACLS guidelines.

Hi Jeff,
there was this question in the ACLS pre-test which says, “you are monitoring a patient with Mobitz type 2 heart block, she has dizziness and BP is 80/40mmHg. She has an IV in place, what is your next action?
a)start transcutaneous pacing
b)give IV atropine 0.5mg
c)start Dopamine infusion at 2-10 mcg/kg/min and then titrate
d)administer sedation & bein immediate transcutaneous pacing at 80/min
e)give atropine 1mg IV
I chose start transcutaneous pacing, as it is stated in ACLS manual “donot rely on atropine for Mobitz 2 and 3degree heart blocks”. But again it also says that 1st line of treatment is Atropine.
The answer in the pre test was to give IV atropine 0.5mg.
Can u pls explain the rational for the answer.

This was a tricky one given the information given on Pg. 111. Which says “Do not rely on atropine in Mobitz type 2 second or third-degree AV block or in patients with third-degree AV block with a new wide QRS complex.
The rational I believe would be “don’t rely on it.” This does not mean that it cannot be given, but it should not be relied upon. I was not sure because I know that in some situations with where 2nd degree or 3rd degree heart block is being caused by an ongoing MI.
I asked by brother who has more experience with these types of situations in the ER and this is what he stated:
I know just the question she is referring to. The AHA ACLS book states that Atopine should be administered first. I believe the rational is that it takes a bit of time to set up the transcutaneous pacemaker. In the meantime give the pt. 0.5 mg of atropine. In general I agree with this and would not hesitate to administer atropine 0.5 mg first. I attached a nice research study that describes the effects of atropine when given to Pt’s with acute MI.
So it sounds like it is ok to give atropine in these situations, but don’t rely on it and move to TCP as soon as you can.

The night before I just did the megacodes and pretest prep and it covered all the main points from the handbook.
