    @Jeff The patient converted with amiodarone, so its the best option for the maintenance infusion. If they converted on lidocaine it would be used for maintenance.

    I fail to find the recommendation to use post-arrest antidysrhytmic infusions in the 2010 guidelines. It is not mentioned under Part 9: Post-Arrest Cardiac Care. Instead this quote from CIRCULATION Part 10:ACS… page S804, states: “Following an episode of VF, there is no conclusive data to support the use of lidocaine or any particular strategy for preventing VF recurrence.” Under Part 8, it is listed as a “consideration” after conversion of stable VT, in the tachycardia algorithm and in the drug tables. Can you tell me specifically where this recommendation is located, as I must be overlooking it.

      Amiodarone maintenance infusion is mentioned on pg. 165 of the AHA provider manual and Lidocaine maintenance infusion is mentioned on pg. 167 of the AHA provider manual. They do not say specifically when to use or not use the maintenance infusion and I could not find specifics for maintenance infusions.
      The only thing mentioned was the maintenance dosing.

      Personally, I have seen both used during cardiac arrest as an antiarrhythmic. These times when used were followed by a maintenance infusion of each medication respectively. Outcomes in these situations were positive.

    QUESTION ONE:
    If she is unresponsive and HR is bradycardia at 25, wouldn’t the correct answer be chest compressions.
    Isint circulation more important than breathing in the 2010 guidelines?

      Hi Al,
      Thanks for the comment. The patient has a carotid pulse. This is symptomatic bradycardia which is treated with the bradycardia algorithm. After implementing measures for support of the airway, atropine should be the first treatment of choice.

    I believe anti arrhythmic medications such as amiodarone drips are no longer indicated post ROSC in 2010 AHA ACLS. Please correct me if I’m wrong.

      Antiarrhythmic infusions (amiodarone/lidocaine) are indicated in the post arrest period if they are give during a code. The use has definitely been de-emphasized. However, in the 2010 AHA ACLS manual, you can find information about maintenance infusion of amiodraone on pg. 165 and maintenance infusion of lidocaine on pg. 167.
