in this scenario you witnessed pt going into asystole so per new protocols arent you supossed to shock first then do CPR?

Hi Melissa,
A shock is not indicated in asystole algorithm. There is no electrical activity in the heart during asystole. Electrical activity is needed for an electrical shock to be effective. Shock in asystole will always fail. CPR must be established, then administration of epinephrine is indicated. Check out the Asystole Algorithm Section

I’m thinking you may not want to start compressions since the patient has a pulse. Airway/O2 support may be the better way to go while attempting to raise the heart rate.

Using the new 2010 guidelines, the first thing to do in this case would be to start compressions first (CAB).

The patient does have a carotid pulse. According to the 2010 AHA provider manual actions should be tailored to the situation.
The patient has a pulse and a is showing signs of poor perfusion. Start with the bradycardia algorithm.
In the green box of the bradycardia algorithm at the top the first intervention is “maintain patent airway, assist breathing as necessary. If the patient becomes pulseless then CAB.

megacode 2, question 16: isnt the answer shock 200J + epinephrine 1mg/vasopressin 40 IU amiodarone is considered after one more round of 5-set HQ-CPR.

Amiodarone is given after the 3 shock in the VT/VF pulseless arrest algorithm.
