I am an oral and maxillofacial surgeon and with my dental background do not have the same medical training as most of you. I was taking the AHA pretest, and I could not deduce the rationale behind tHe answer for this question.. could you please clarify?
Q: A patient is in refractory V Fib. High quality CPR is in progress, and shocks have been given. One dose of epi given after second shock. An anti arrhythmic was given immediately after third shock. What drug should be given next?
I chose the option “repeat the anti arrythmic”
But the right answer indicated was “repeat epi”
The VF/pulseless VT algorithm doesnt mention epi after amiodarone?
are we supposed to choose epi because epi is supposed to be repeated every 3-5 minutes and we must assume that that much time would’ve elapsed adn hence repeat epi?
or ahould we go back to the CPR wit epi step after amiodarone is given and rhythm check is done and repeat the algorithm from there?

I went in and took that quiz. The choices were “repeat the antiarrhythmic drug”
or “second dose of epinephrine 1mg.” In this case I would have chosen the epinephrine because it sounds like you are repeating the same dose of the antiarrhythmic drug. It did not say anything about giving 150mg rather than 300mg. In this case, I would have chosen the epinephrine.
Technically, if they would have said “give 150mg amiodarone” or “give 1mg epinephrine,” either one could have been true.
Hope this makes sense.

I just got certified! With the AHA manual not being much of a help, I first turned to the usual books (Dubin’s EKG and Aehlert’s ACLS study guide). They provide a sound theoretical background, but your site helped me tremendously by letting me actually think about the algorithms. Great preparation for the megacodes.
