    So since when did they start allowing people to use the algorithm cheat cards during ACLS mega code Check off?? I just rectified for the 4th time and this time I was at a different facility from where I had tested previously and I was surprised that they allowed people to use their algorithms during the skills Check

      It depends on where you test and even who does the testing. Personally, I think that those certifying should know the material without the ACLS algorithm cards. What if the cards are not available. Will those that are certified be able to perform their duties as an ACLS provider. I think that it is unwise to use these cards for certification.

    I bought the 2 week cram package 4 days prior to my mega code and written test. It was very helpful. The written test questions are similar to the ones that you have on your written test. The mega code videos help you pull everything together. This is a total package deal. Jeff, you done an excellent job. I will recommend this to all who take ACLS. Thank so much.

    I thot for Vtach you give synchronised shock. Why unsynchronised? also, if the rhythm is shockable but pt is pulseless, if we don’t know how long he has been down, first start CPR right? but if its a witnessed vtach conversion – shock first. thanks, Jeff.

      Pulseless VT is treated with unsynchronized shocks. VT with a pulse would be treated with synchronized shocks unless the defibrillator will not sync with the rhythm.
      To your second question, if the arrest is not witnessed then start with CPR for 5 cycles and call for the crash cart. If the arrest is witnessed then shock asap.

        There is no such rule. The left branch of the pulseless arrest algorithm which involves Pulseless VT and VF is the most important algorithm and shocks are one of the most important interventions of the algorithm.

        You do not shock asystole. Which is basically no electrical activity at all. I recommend that you go through the information in the knowledge base of the website to gain further understanding. The knowledge base is located in the left hand navigation.

      In Question #3 since the patient has a SVT with a pulse, is showing signs of poor perfusion, and other measures have failed to convert the rhythm, synchronized cardioversion should be attempted.

    Hi Jeff,

    I really appreciate all of the hard work you’ve put into this site, it’s been immensely helpful! I just have a question regarding VF/VT and PEA. I’m confused about which algorithm to pursue when I see pulseless VT or VF. Are these rhythms shockable even if they’re pulseless? In one of the megacodes, I was ready to begin CPR but was instead told to shock even though no pulse was present. I could be very wrong, but I thought that PEA was applicable to any rhythm without a pulse.

      Apologies, I’ve just re-read the information for VF and Pulseless VT. I see my mistake!

      Once again, very comprehensive site. I was very nervous about doing my ACLS and after going through the megacode videos and quizzes, I don’t feel nearly as intimidated as I did when I received the ACLS manual.
