    I HAVE MY ACLS EXAM IN A MONTH AND I JUST PURCHASED THIS SITE AND I AM NOT SURE I DID THE RIGHT CHOICE. I REALLY THINK THERE SHOULD BE AN EXPLANATION OR RATIONALE FOR WHY THE ANSWER IS CORRECT. I HAVE VERY BAD TEST ANXIETY AND I REALLY NEED TO PASS THIS EXAM. THANKS

    Very nice site. I suggest it to my paramedic students to prepare for their megacode testing.

    One suggestion – keep in mind the “manufacturer recommendations” for defibrillation. Have a specific number for defib without the monitor type is a little difficult to answer.

      I answered the questions without reviewing check how much knowledge I have after no practice and use since 2002. I will highly recommend this for ACLS takers.

      In the post arrest phase once ROSC is established but the pt. does not follow commands, it is recommended that therapeutic hypothermia be initiated. See page 77 of the AHA ACLS provider manual.

    Jeff, i need help, so on q 3 of this narrow SVT scenario, why do you go to cardioversion and not defib? I picked defib due to fact pt had really weak pulse, and i thought pt had converted to a pulsless v. Tach. Is that fair reasoning? Can that occur? i am confused. Thank you for your help. Great site!!!!

      My other question is from q 7 on this scenario. Why does the third defib only use 200J as opposed to 360? I read that you start with 200, 300, and then 360 on biphasic machines. Thanks!

        If you had a biphasic defibrillator goes past 220 J then you could continue to increase. Many biphasic defibrillators do not go past 220. With biphasic, you would normally start with 120 J and then increase in a stepwise fashion.

        Coughing forcefully is a simple form of vagal maneuver. The idea is to increase intrathroacic pressure which will stimulate the vegus nerve. This can result in slowed conduction of electrical impulses through the AV node of the heart.

    We had a kid (approximately 12 yrs old) present to our ER in SVT, instead of using drugs and vagal maneuvers we dunked his face in a bucket of ice twice and it immediately & effectively brought the heart rate down. We observed him for a period of time and the kid was fine. Worked wonders! Young army doctor’s idea!
