As a nurse. I can not allow to do vagal maneuvers? If yes How can I do it
Jeff with admin. says
You can have the patient blow through a straw, bear down, or try and blow the plunger out of a syringe. All of these interventions should be sufficient in producing a vagal response.
You should not perform a carotid massage without a direct order. I personally would not to a carotid massage without the physician in the room. Too much liability.
Kind regards,
Jeff

IF YOU READ THE ADENOSINE PACKAGING IT STATES IT CAN BE GIVEN IN 3 DIFFERENT DOSES? IS THIS WHAT WE WILL SEE IN THE FUTURE SOON FROM THE AMERICAN HEART ASSOCIATION?
Jeff with admin. says
I hope that AHA will clarify a little better on this. At this time, the algorithm has 6 mg adenosine and if this does not work repeat 12mg adenosine. There is no option for another 12mg dose of adenosine.
In my own professional opinion, I would give if the 6 mg and the first 12 mg fails, I would repeat the 12mg dose.
Strictly speaking the guidelines state: 6mg x 1 and may repeat 12mg x 1.
Kind regards,
Jeff
EH says
we give 6mg followed by 12 mg then another 12mg if it didn’t convert after the first 2 doses
Jeff with admin. says
I personally would give a 2nd 12mg because I have not found any rational for not giving a 2nd 12mg dose.
Kind regards,
Jeff

thank you very helpfull
JMac says
The standard in Canada with the Heart and Stroke Foundation is 1.5 mg/kg for the first dose in this scenario (V-Tach arrest) 2nd (and 3rd if needed) dose is 0.75mg/kg.
I failed to realize this during my Cdn megacode and was unsuccessful in my first practical attempt at the megacode. Oops!
Your simulator is very helpful but us foreigners must remember to translate to local protocols.
I’ll try again tomorrow.

Hi JMac,
I was not aware of the difference regarding the administration of amiodarone. Thanks for making us all aware.
